CONGRATULATIONS! Welcome to the Caring Hearts and Paws Rescue Family!! We are happy you have been approved to adopt your next dog! Below you will find your Agreement and some very important information to look over. Please sign ( digitally) upon completion of reading and send back within 24 hours.

Terms & Conditions*
- Adopter/s understand that if the agreement is not signed and returned within 24 hours, Caring Hearts and Paws will move on to the next potential adopter for the dog they are adopting.
- Adopter/s understand that if for whichever reason they choose to no longer adopt Rescue Animal prior to entering your care, Caring Hearts and Paws Rescue withholds 100.00 from the adoption fee.
- Adopter/s understand that Payment is made Via quickbooks invoice, and that will be sent over as a separate email.. Adopter/s agree that they will include the name of my new dog/puppy in the notes if it is not noted on invoice.
- Adopter/s are 21 years old or older or has parent/guardian approval. If under 21, Parent or Guardian needs to sign as well. Agreement will not be complete without.
- Adopter/s agree that they are adopting Rescue dog/puppy for them and agree to not give it away, sell, or trade rescue animals. Even as a gift to a friend or family member. Adopter/s agree that they will not abandon nor take Rescue to a shelter if Adopter/s no longer keep the adopted animal. Adopter/s understand that they must notify our organization that they can no longer take care of or keep said Rescue animal.
- Adopter/s agree that they reside in a dwelling that permits dogs and acknowledged this adoption will not be a violation of any existing residential agreement. If I move to a residence that does not allow pets, I agree to reach out to the Caring Hearts and Paws Rescue and discuss the options of rehoming.
- Adopter/s agree that this animal will be treated as a family member with loving care and affection and will do their best to ensure the rescue’s safety, health, and overall well-being. Health and well being inside the home as well as outside the home. Home environment will be a safe environment for said rescue dog/puppy.
- Adopter/s agree to provide a well suitable home and yard for Rescue animal. Rescue animal will live inside the home and will not be isolated from the family. Rescue animal will be provided adequate exercise. Rescue animal will never roam free, be chained or tied up, kept continuously in a garage, yard, patio, balcony, or pen. Rescue animal will not be left outdoors when no one is home.
- Adopter/s agree that Rescue Animal will not be attack-trained, or receive any physical, mental or emotional abuse nor be used for any purpose other than companionship.
- Adopter/s agree that the Rescue animal will go to a licensed veterinarian when shots/vaccines are due, but in no event later than one year from the last visit. Adopter/s will provide all required and or needed veterinary care, including rabies, shots/vaccines.
- Adopter/s agree that Rescue animal will be given heart-worm preventative for at least 12 consecutive months upon adoption.
- Adopter/s agree to comply with the law regarding ANY and ALL of animal rights and animal protection laws.
- Adopter/s agree If Rescue animal (dog or puppy) is not spayed or neutered that by signing this contract, adopter/s will have them spayed and or neutered by no later than recommendation by licensed veterinarian and will submit certification to Caring Hearts and Paws Rescue. If dog/puppy is not spayed or neutered within 4 months of adoption and Certificate sent to Caring Hearts and Paws rescue, Adopter/s understand this contract will be void and it is considered a breech of contract. The dog/puppy will be returned/Surrendered back to Caring and Paws Rescue.
- Adopter/s affirm that no member of the household has been convicted or an animal welfare law violation such as neglect, cruelty, abandonment, etc.
- Adopter/s agree that they will ensure proper licensing of rescue animal applicable to local and state licensing statues. Adopter/s accept responsibility and ownership of said Rescue animal at their own risk and adopter releases organization and its agents from any and all liability arising out of possession and ownership of rescue animal.
- Adopter/s agree to assume total responsibility for the rescue animal as of the date of this contract. Organization and its agents will not be held responsible for any damages or expenses (veterinary or other) incurred during Adopter’s ownership of rescue animal.
- Adopter/s agree that the Rescue animal's known background and medical history have been discussed with them and they understand that our organization has made no misrepresentation concerning the health, condition, training, behavior, or temperament of said rescue animal.
- Adopter/s agree that If for any reason a rescue dog runs off the property/away, Caring Hearts and Paws will be notified immediately.
- Probationary Period Provisions: Should the adopter decide to NOT finalize the adoption prior to the rescued/adopted animal arriving at their home, they will receive a refund minus $100.00. If Caring hearts and Paws rescue Inc is not informed of this decision, the adoption is considered to be finalized. Once the adoption is finalized there is no refund for dogs that are returned. Should the adoption of the Animal fail, the Adopter must send an email to Caring hearts and Paws Rescue stating the reason for the adoption failure and a request to relinquish care back to Caring hearts and Paws Rescue Inc. Upon request Caring hearts and Paws rescue will send the Adopter a Surrender Form, which must be signed and returned immediately.
- Any Payment that gets kicked back or there is a discrepancy with adoption payment, the adoption of said animal will be null and void.
- $50.00 deposit fee will be reimbursed to adopted FAMILY when spay and Neuter certificate is submitted.
